PLEASE INCLUDE THE FOLLOWING WITH YOUR REGISTRATION APPLICATION
All of the following documents must be originals:
- Completed registration form
- Birth certificate (issued by the Registrar of Civil Status)
- Proof of education (diplomas, academic transcripts, certificates)
- Recent driving record from the SAAQ
- SAAQ certificate stating that you passed the vision test
- SAAQ certificate stating that the medical report was processed
- Proof of residence in the case of persons born outside Quebec
- Recent photo
